Reasons to complete a Voter Registration Application Form:

• New Registration
• Name Change
• Address Change
• Signature Update
• Political Party Affiliation (can also be filed with a Party Affiliation Declaration Form and mailed to the address below)
• Vote by Mail (can also be filed with an Application for Vote By Mail Ballot and mailed to the address below).

The registrant must complete a Voter Registration Application Form and mail or deliver to:

Voter registration forms and Vote By Mail applications are also available in the Municipal Clerk's office. The registration deadline to vote in the next election is 21 days prior to the Election Day.
